About Us

Baltimore PACE

Edenbridge PACE is a health care and service support program designed to assist older adults. With Edenbridge PACE’s assistance, participants can stay in their own homes and communities at the time of enrollment. However, the individual must be able to live safely in a community setting without jeopardizing his or her own safety.

Advantages of Edenbridge Pace

The PACE interdisciplinary team (IDT) guides all aspects of the participant care through the development of an individualized care plan that combines medical, physical, emotional, psychosocial, and therapeutic supports and interventions to allow participants to remain in their own homes and communities.
