About Us
Edenbridge PACE is a health care and service support program designed to assist older adults. With Edenbridge PACE’s assistance, participants can stay in their own homes and communities at the time of enrollment. However, the individual must be able to live safely in a community setting without jeopardizing his or her own safety.
Pace Care History
The national Program of All-inclusive Care for the Elderly (PACE) is an innovative system of care designed to meet the needs of frail elderly individuals. By combining medical care, community- based home and health care services, and day programs PACE programs help the aging stay safely in their own homes and communities. There are PACE programs throughout the United States.
Why Edenbridge Pace
Edenbridge PACE provides a wide variety of services twenty-four hours a day, seven days a week, three hundred sixty-five days a year.
Our team meets with each participant to individually assess their goals of care to tailor the specific services we provide and coordinate to meet their unique needs.
PACE care team members:
- Coordinate all health care services
- Assess participants’ needs regularly
- Develop and monitor participant care plan
- Provide direct services to participant
- Coordinate outpatient services and inpatient hospitalizations
- Contract with specialty physicians and providers
- Support community-based living with assistance
