Participant Rights And Responsibilities Upon Disenrollment
Participant/Responsible Party/Family Member:
Edenbridge PACE understands that you wish to disenroll from the Edenbridge PACE program. The following is a guideline to help you through the disenrollment process as smoothly as possible. We suggest that you work with your Edenbridge PACE social worker who can help you with this process.
It is important to remember that although you can disenroll from Edenbridge PACE at any time, the disenrollment becomes effective at midnight on the last day of the month following the date we receive notice of your intent to disenroll.
You cannot be enrolled in Edenbridge PACE for part of a month and enrolled in the regular Medicare or Medicaid system for part of that same month.
You cannot disenroll from the Edenbridge PACE program at a Social Security Office.
Edenbridge PACE will help you to coordinate getting out of the Edenbridge PACE program and back into the Medicare and Medicaid systems. Once again, it is important that you work with your social worker so that you can get the services that you need.
Medicare: (If you are eligible for Medicare)
Once you are disenrolled from Edenbridge PACE, you will automatically be re-enrolled into fee-for-service Medicare. If you had an HMO before you came to Edenbridge PACE, you will need to call the plan to sign up again. We can help you with this.
Medicaid: (If you are eligible for Medicaid)
Edenbridge PACE will coordinate the disenrollment date with the [SAA] and reinstate you in Medicaid programs for which you are eligible.
If you are moving to a new county, you will need to reapply for Medicaid in the new county. Please let your social worker know so you can receive help in getting the paperwork completed.
Choosing a New Doctor
Once you have made the decision to disenroll and prior to disenrollment, you will need to pick a new primary care doctor. You will need to make an appointment as soon as possible in the month your enrollment is effective. We can help you with this.
Edenbridge PACE will give you a 30-day supply of medicine, so that you will have time to meet your new doctor and get new medicines ordered. If you wish, we will send your records to your new doctor at no cost to you.
Equipment and Supplies
After your disenrollment becomes effective, Edenbridge PACE will come to your home and pick up the equipment that we gave you to use while you were in the program. Edenbridge PACE will help you to have these items replaced by your new provider before we pick up equipment that Edenbridge PACE has given you.
Discharge Plan
The Edenbridge PACE team will work with you to put together a discharge plan and help you in getting some things set up. You will receive a written copy of your discharge plan.
