About Us
Edenbridge PACE in West Baltimore, Maryland uses a combination of medical and emotional support services, including physical, psychosocial and emotional evaluation, so older adults may stay safe, comfortable, and healthy at home.
The Program of All-Inclusive Care for the Elderly (PACE) provides a specialized focus on managing the care and needs of the older adults through concentrating on the values and personal autonomy of each participant.
The PACE interdisciplinary team (IDT) guides all aspects of participant care through the development of an individualized care plan and
interventions to allow participants to remain in their own homes and communities as long as possible.
Edenbridge PACE is a health care and service support program designed to assist older adults. To be eligible for PACE, participants must be determined to be able to live safely at home or in the community with support. If the interdisciplinary team determines the participant is no longer able to live safely at home, participants may be transferred into outside facilities as approved by the interdisciplinary team. Edenbridge PACE provides, coordinates and oversees all needed preventive, primary, acute, and long-term care services.
