Eligibility Criteria

To be eligible for Edenbridge PACE care, the participant must meet the following qualifications:

  • 55 Years of age or older
  • Qualify for nursing home care according to Maryland’s Medical Assistance pre-screening program for eligibility
  • Live within our service area
  • Live safely within your community

By choosing to enroll in Edenbridge PACE, participants must receive all needed healthcare services, including primary care and specialist physician services (other than emergency services), from Edenbridge PACE or a person or company approved by the PACE interdisciplinary team. Edenbridge PACE guarantees access to services, but not to a specific provider.
